Template of Notice for Obtaining Consent to Personal Data Processing

The Freedom of Information Center of Armenia has developed a template for the notice used to obtain consent for the processing of personal data. The document defines the key components of the consent notice and provides a detailed outline of the content required for each element.

The following sections are included in the document:

  • Surname, first name, and patronymic of the data subject

  • Legal grounds and purpose of personal data processing

  • List of personal data to be processed

  • List of actions involving personal data for which consent is requested

  • The circle of persons to whom the personal data may be transferred

  • Name (surname, first name, patronymic, and position) and location or registered (actual) address of the processor or their representative requesting the consent

  • Information about the data subject’s rights to request correction, deletion, suspension of processing, or other actions related to the processing of personal data

  • Duration of the requested consent, as well as the procedure for its withdrawal and the consequences thereof
