The Freedom of information Center prepared a sample of an information request for all those people who want to get information. The sample of written information request prepared by the FOICA (https://www.foi.am/en/information-request-form/) is already posted on websites of more than 10 state bodies, for example www.president.am, www.gov.am,  www.police.am, www.mta.gov.am, www.moh.am, websites of all 10 regional administrations of the RA, etc. The sample of an information request helps people to formulate legally competent requests.
